If you're looking to complete your practicum at CSDM, make sure to complete your WHMIS certification, download and sign the Confidentiality Contract, and have your physician or NP complete your Immunization Form. Then, proceed to the next step where you will be able to send in your application for the practicum. Please note that this pharmacy now accepts exclusively students who complete a FAMA-enhanced Background Check. We do not accept any other certificate from any police service anywhere in the world. The cost of the FAMA-enhanced Background Check is the responsibility of the student and costs $100. On receipt of your application, CSDM will contact you for the payment of the $100 fees for the check. You will have the option to pay over the phone by credit or debit card. To proceed to the next step in your application, please click here. THE APPLICATION MUST BE COMPLETED AND SENT AT LEAST 30 DAYS BEFORE THE PRACTICUM IS DUE TO START. LATE APPLICATIONS WILL BE REJECTED!
